Practitioner Program Description
Trustmark's Condition Management
Program is administered by Alere for Trustmark Insurance
Company. This new condition management program provides support,
information, and services to help eligible* members with asthma,
CAD, CHF, COPD, and diabetes better manage their condition. It is
offered at no cost to your patients through Trustmark Insurance
Company.
Trustmark's Condition Management
Program is designed to complement your prescribed treatment plan
and clinical decision-making. Through this program, our care coordinators
will encourage, support and educate members according to your
recommended plan of care. You will receive feedback through summary
reports of our interactions with your patients.
As a participant in the program,
your patients will receive the following:

Periodic health risk assessments to determine current health status,
educational needs and degree of control of their condition

Comprehensive education regarding his or her specific condition

Educational materials (available in English as well as Spanish)

Specialty nurse follow-up calls to provide education and support
according to your plan of care;

Toll-free access to a support line 24 hours a day, seven days a week.
The program has an ongoing process to review significant new
findings. Each year the program updates its guidelines to the most current
recommendations as published by the following organizations: the American
Diabetes Association (ADA), the American Heart Association (AHA), the American
College of Cardiology (ACC), the National Heart Lung and Blood Institute
(NHLBI), the Global Initiative for Chronic Obstructive Pulmonary Disease; the
Institute for Clinical Systems Improvement (ICSI) and others. These guidelines

If you have questions about the program, please call Alere at
1-800-283-7159. You may also use this toll-free number to file a grievance or
complaint or to provide feedback about the program. We are pleased to provide
the program to your patients during our regular hours, Monday through Thursday,
7:00 a.m. to 10:00 p.m.; Friday, 7:00 a.m. to 8:00 p.m. and Saturday, 8:00 a.m.
to 5:00 p.m. Central Time. After-hours support is available 24 hours a day,
seven days a week.
