Participant Program Description

Trustmark's Condition Management Program provides support, information and services to help you better manage your conditions. This condition management program will help you improve your ability to carry out daily activities and improve your overall quality of life.

Program participants will receive the following:

• A health risk assessment twice a year to determine your health status
  
  
• Periodic educational materials (available in Spanish as well as English) following each telephone contact
  
  
• Toll-free access to a clinician available to answer complex medical questions, 24 hours a day, seven days a week
  
  
• Toll-free access to a care coordinator who will review your health status and answer your questions

This is a patient education program. If you have urgent information or concerns regarding your healthcare, please contact your healthcare provider. In emergency situations, call 911 or go to the emergency room.
