Welcome to the Trustmark's Condition Management Program administered by Alere for Trustmark Insurance Company. This Condition management program provides support, information and services to help you better manage your condition and improve your quality of life. It is offered at no additional cost to you through your health plan program.

This program includes:

• Access to a Trustmark Condition Management Program care coordinator who will periodically call you to review your health status, answer your questions, and provide encouragement.
  
  
• A staff of professionals who will be available to answer more complex clinical questions about asthma, coronary artery disease, congestive heart disease, chronic obstructive pulmonary disease, and diabetes.
  
  
• Helpful educational materials that will be sent to you. These educational materials are also available on this Website in the Information Center.
  
  
• Health assessments, which will be used to determine your overall health status and to identify areas where you may require additional support and education. These assessments are also available on this site in the Assessment Center.

You may enroll in this program by calling Alere at 1-800-283-7159. You can call, Monday through Thursday, 7:00 a.m. to 10:00 p.m.; Friday, 7:00 a.m. to 8:00 p.m. and Saturday, 8:00 a.m. to 5:00 p.m. Central Time. After-hours support is available 24 hours a day, seven days a week.
